What Is Pico Salax?

Getting a colonoscopy, endoscopy, or bowel surgery scheduled means one thing has to happen first — your colon needs to be completely clear. Pico Salax is the preparation kit designed specifically for that job.

Each box contains two cranberry-flavoured sachets, taken at separate times the day before your procedure. Together, they work to thoroughly cleanse the entire bowel so that your doctor gets a clear, unobstructed view during the examination. A properly prepped colon isn't just about comfort — it's what allows polyps, lesions, and other abnormalities to actually be seen and removed safely.

Pico Salax is one of the most commonly prescribed bowel prep solutions in Canada. The cranberry flavour makes it more palatable than many older prep formulations, and the sachet format is straightforward to use at home following your clinic's specific instructions.

How It Works

Pico Salax uses a triple-action combination that tackles bowel cleansing from two directions simultaneously:

Sodium Picosulfate acts as a stimulant laxative. Once it reaches the colon, gut bacteria convert it into an active form that stimulates the bowel wall directly, triggering contractions that move contents through and out.

Magnesium Oxide and Citric Acid react together when dissolved in water to form magnesium citrate — an osmotic agent that draws water into the bowel from surrounding tissue. This softens and liquefies stool, making it easier to pass, while also creating the fluid volume needed to flush the colon clean.

Together, these three ingredients produce a thorough cleanse that typically begins within 2 to 3 hours of the first sachet. Most people complete the majority of their bowel movements before the second sachet is even due.

How to Use cc — Step by Step

Your clinic or doctor will provide specific timing instructions for your procedure date — always follow those first. The general protocol looks like this:

Preparing each sachet:

• Pour the contents of one sachet into a glass containing 150 mL of cold water

• Stir until fully dissolved — the mixture will become warm briefly as magnesium oxide and citric acid react

• Wait for it to cool before drinking

• Drink the full glass

Standard two-dose schedule:

• First sachet: typically taken in the early afternoon the day before your procedure (around 2–4 PM)

• Second sachet: typically taken 6 hours later, in the early evening (around 8–10 PM)

Fluid intake is critical: After each sachet, you must drink at least 5 to 6 additional glasses of clear fluids over the next few hours. This keeps you hydrated and helps the prep work effectively. Approved clear fluids include water, clear broth, apple juice, white grape juice, clear sports drinks, and plain gelatin — nothing red or purple coloured.

Staying close to a bathroom after taking each dose is strongly recommended. Effects begin within a few hours and can be significant.

Side Effects

Common and expected:

• Loose, watery stools — this is the intended effect

• Abdominal cramping and bloating

• Nausea

• Headache

• Fatigue

These are normal parts of the bowel prep process. Staying well hydrated and resting helps manage them.

Contact your doctor right away if you experience:

• Lightheadedness or feeling faint

• Severe nausea or vomiting that prevents you from keeping fluids down

• Significant bloating or severe stomach cramps

• Rectal bleeding

• Confusion, irregular heartbeat, or seizure

• Very little or no urination — a sign of dehydration

• Difficulty swallowing or drinking

Severe electrolyte imbalance is the most serious risk with any bowel prep — this is why adequate fluid intake before, during, and after using Pico Salax is non-negotiable.

Who Should NOT Use Pico Salax

Do not use Pico Salax if you have:

• Kidney disease — magnesium is processed through the kidneys; impaired kidneys can't clear it safely

• Bowel obstruction, ileus, or colitis

• Toxic megacolon or intestinal perforation

• Slow digestion (gastroparesis)

• A known allergy to sodium picosulfate, magnesium oxide, or citric acid

Tell your doctor before using if you have a history of:

• Electrolyte imbalances (low potassium, sodium, or magnesium)

• Heart problems or are on heart medications

• Seizures or are currently taking anti-seizure medications

• Ulcerative colitis or GERD

• Recent use of sedatives (diazepam, lorazepam, alprazolam)

Pregnancy and breastfeeding: Safety has not been fully established. Speak with your doctor before using.

Age restriction: Not approved for anyone under 9 years of age.

Drug Interactions — Timing Matters

Pico-Salax can affect how certain medications absorb on the day it's taken. If you use any of the following, take them at least 2 hours before or no less than 6 hours after each Pico-Salax dose:

• Antibiotics

• Digoxin

• Iron supplements

• Chlorpromazine

• Penicillamine

Always tell your doctor and clinic about every medication you take — including vitamins and supplements — before your prep day.

Store sachets at room temperature, away from heat and moisture. Keep in original packaging until use. Keep out of reach of children.
