Zoryve Foam (Roflumilast 0.3%): A Guide by Where It Shows Up

Seborrheic dermatitis doesn't behave the same way on every part of the body. The scalp version looks nothing like the version that shows up along the sides of the nose, and the chest version has its own quirks entirely. Zoryve Foam was actually designed with that in mind — it's a leave-in foam, not a wash-off shampoo, specifically because seb derm rarely stays in one tidy spot. Here's how it works, region by region, plus everything else worth knowing before you fill the prescription.

What's Actually in the Can

Zoryve Foam's active ingredient is roflumilast 0.3%, a phosphodiesterase-4 (PDE4) inhibitor. PDE4 is an enzyme that helps drive the inflammatory signaling behind seborrheic dermatitis; roflumilast blocks it, which calms the redness, flaking, and itch without touching the skin the way a steroid does.

That last part matters more than it might sound. Zoryve is not a topical corticosteroid. It doesn't come with the thinning-skin, rebound-flare risks that make dermatologists nervous about long-term steroid use on the face or around the eyes. That's a big part of why the FDA approval — the first new mechanism for seb derm in nearly two decades — was treated as a genuinely big deal in dermatology circles.

Scalp

This is where most people first notice seb derm: flaking that looks like stubborn dandruff, sometimes with visible redness at the hairline or a tight, itchy scalp that gets worse with stress or cold weather.

• Apply to a dry scalp — not right after a shower, not on damp hair
• Part the hair as needed to reach the skin directly; the foam is designed to spread through hair without needing to rub for minutes
• You do not need to wash it out — it's a leave-in product
• Once daily is the studied and approved frequency; more isn't better here

Face — Eyebrows, Nasolabial Folds, Around the Nose

The face is often the part patients are most anxious about treating, because it's also where steroid creams are least appropriate for long-term use. This is arguably where Zoryve's steroid-free profile earns its keep.

• Use a thin layer only — this is delicate skin and a little foam goes a long way
• Common trouble spots: the eyebrows, the creases beside the nose, and the hairline just above the forehead
• Keep it away from the eyes, inside the nose, and the mouth — Zoryve is for external skin use only
• Because it's not a steroid, it's considered appropriate for longer-term, ongoing use on the face where a corticosteroid typically wouldn't be

Ears and Behind the Ears

Small area, easy to under-treat. Flaking and redness in the ear canal opening or the crease behind the ear gets missed because it's hard to see in a mirror.

• A fingertip's worth is plenty for this whole region
• Don't apply inside the ear canal itself — stick to the visible skin of the outer ear and the fold behind it

Chest and Upper Back

Seb derm on the trunk tends to show up in a butterfly-shaped or scattered pattern across the central chest and upper back, often described as more persistently itchy than the scalp version.

• Larger surface area means it's worth applying a slightly wider (but still thin) layer rather than concentrating it in one spot
• Let the foam dry before getting dressed to avoid transferring product to clothing

Beard Area and Facial Hair

Seb derm underneath a beard is notoriously easy to miss because the hair hides the flaking until it's fairly advanced.

• Apply the same way as scalp application: part through the beard to reach skin directly, don't just pat it on top of the hair
• Rub in completely so it isn't sitting on the hair shaft rather than the skin underneath

One Warning That Applies Everywhere: It's Flammable

This is the single most important safety note across every application site, and it's easy to overlook because it doesn't sound like a typical medication warning. The propellant in Zoryve Foam is flammable. Avoid fire, flame, and smoking during application and for a period immediately afterward, on any part of the body you're treating — this includes lighting a stove, using a lighter, or standing near an open flame shortly after applying.

Who Shouldn't Use It

Zoryve Foam is approved for seborrheic dermatitis in patients 9 years of age and older. It's not recommended for people with moderate to severe liver impairment (Child-Pugh B or C), since roflumilast is metabolized through the liver. If you're pregnant or breastfeeding, talk to your doctor — safety data in pregnancy is limited, so this is a discuss-first, not skip-automatically, conversation.

Side Effects Worth Knowing About

Most people tolerate Zoryve Foam well, with minimal stinging or burning on application compared to some other topical treatments. The side effects reported most often in clinical trials include:

• Headache
• Nausea
• Nasal or sinus symptoms (runny or stuffy nose)
• Diarrhea and trouble sleeping, though both were reported in fewer than 1% of patients

If you notice significant irritation, hives, or swelling at an application site, stop and contact your doctor — that combination was the leading reason patients discontinued in trials.

What the Evidence Actually Showed

In the pivotal STRATUM trial and a companion Phase 2 study covering 683 adolescents and adults with seborrheic dermatitis, roughly 77% of people using Zoryve Foam reached "Clear" or "Almost Clear" skin by week 8, compared to about 53% of those using the inactive foam base. Improvement over the vehicle showed up early in the trial period, not just at the final assessment — which lines up with what many patients report anecdotally: less itch within the first couple of weeks, with fuller clearing by the two-month mark.
